Program Purpose and Objectives

Today's healthcare practitioners need more than just clinical expertise; they must also understand education, research, and leadership. This set of three non-clinical modules is designed to provide basics and practical knowledge that can be applied in your healthcare delivery setting. Within each set, we will work to cover specific issues of interest to you, and will do individual and team activities to apply new knowledge.

There are three unifying themes to the set of interactive clinical lectures.
  1. Clinicians will learn evidence-based details regarding management of common impactful issues in primary care such as type II diabetes, hyperlipidemia, and contraception.
  2. Clinicians will learn general patient-oriented schemes to evaluate the strengths and weaknesses of various patient care options.
  3. linicians will learn and use tools which they can keep using back home which provide quick-reference decision support during patient care.

    • Differentiate between learning goals and objectives and construct a learning objective relevant to their own clinical setting.
  2. Education Basics: Connecting the learner and the content.
    • Review and apply the basic traits of adult learners to the content we want them to master.

    • Differentiate between the ADDIE and ASSURE models, and select the best approach depending on your goals.
  4. Connecting Research to Practice: What is research; and what are the building blocks?
    • Define research and identity three basic strategies.
  5. Connecting Research to Practice: Finding useful research outcomes.
    • Identify processes to find useful and relevant research outcomes.
  6. Creating and Managing Teams: What's the Difference Between a Staff and a Team?
    • Differentiate between a staff and a team and discuss strategies to transition from staff to team.
  7. Creating and Managing Teams: How do Teams Become Self-supporting?
    • Define and explore the roles of climate and culture in the clinical setting and they foster team based care.
  8. Pre-Operative Clearance: 30-day perioperative mortality, were it a discreet cause of death entity, would be the 3rd largest cause of death in the US. Many are preventable.

  9. Type II Diabetes Mellitus Medication: Use is widespread of medications which do not improve long-term diabetes outcomes.

    • Make patient-oriented, evidence-based medication decisions according to patient characteristics.
  10. Hyperlipidemia Medications: Nearly all know to use statins. But clinicians need evidence-based guidance on what else to use when statins are not tolerated or not enough.
    • List the categories of hyperlipidemia meds
    • Cite the criteria by which interventions should be judged
    • Apply those criteria to all hyperlipidemia options
    • Learn why statins are prescribed first line
    • Learn what to use when non-statin therapy is necessary
  11. Decisions Making Tools: Many questions large and small arise during busy patient care, yet few answers are sought.
    • Make peace with not knowing everything
    • Learn about good point-of-care decision support apps
    • Use these apps in clinical cases together
  12. Relative Risk Assessment: Too often recommendations made by physicians are black-and-white and based upon relative unclarified risk
    • Analyze studies from the medical literature
    • Quantify risk of options and alternatives

    • Learn what happened in UK when this was done poorly.
  13. Glycemic Control: At best physicians are unclear on how to customize glycemic goals for type II diabetes patients, and at worst they think the goal is to normalize blood sugar, which evidence and ACP guideline do not support

Karen Hughes Miller, PhD, is recently retired as Associate Professor in the University of Louisville School of Medicine (U of L SOM) Office of Graduate Medical Education where her focus was on medical education research and curriculum planning. She oversaw the graduate non-clinical curriculum, and served on the U of L SOM Faculty Development Committee and QI and Patient Safety committees.



Dr. Miller has more than 50 publications and presentations. She has taught adult education, curriculum design, and assessment courses and workshops to undergraduate and graduate students, medical residents and fellows, and faculty. She is also experienced in teaching task analysis to educators and physicians.

Dr. Miller's undergraduate degrees in journalism (with a minor in commercial art) and her master's degree in instructional technology are useful today in creating new teaching and learning materials and platforms. One of her several research agendas is teaching and assessment of very bright learners, and she is currently completing a book chapter on this topic for NOVA publishers.



Dr. Miller is Immediate Past Chair of the Association of American of Medical Colleges (AAMC) Southern Group on Education Affairs (SGEA), and the Immediate Past Chair of the AAMC Research in Medical Education (RIME) program selection committee.

The experience is about more than just the coffee; it's about the sounds of Spanish, the rhythm of salsa music, and the sight of locals debating over a game of dominoes at Máximo Gómez Park. It’s an accessible, genuine cultural immersion.

In the afternoon, witness the city’s stunning contrasts. First, visit the Wynwood Walls, an extraordinary outdoor museum of large-scale murals by some of the world's best street artists. San Juan offers one of the most sophisticated and accessible experiences in the Caribbean. Unlike ports requiring long transfers to see anything of value, Old San Juan greets you immediately upon disembarkation. Your priority should be the two massive citadels guarding the coast. Start at Castillo San Cristóbal, the largest Spanish fortification in the New World, before walking the scenic coastal trail to the iconic Castillo San Felipe del Morro. The sheer scale of these defenses is humbling, and the expansive green lawn leading to El Morro—often dotted with kites—provides a dramatic contrast to the deep blue Atlantic.

Between the forts lies a grid of blue cobblestone streets (adoquines) lined with vibrant colonial architecture. The joy here is aimless wandering, though the heat can be intense; stay hydrated and wear sturdy walking shoes to navigate the hills. For a cultural pause, visit the Cathedral of San Juan Bautista, the resting place of Ponce de León.

Culinary exploration is essential. Skip the generic tourist traps near the pier and head deeper into the city. Seek out mofongo (garlicky mashed plantains) at a local staple like El Jibarito or Café Puerto Rico. While the nearby beaches of Condado are beautiful, they require a taxi and contend with traffic. Unless you have visited before, we advise dedicating your day entirely to the history and atmosphere within the city walls, finishing perhaps with a Piña Colada at Barrachina, right in the heart of the old town.

Charlotte Amalie is the heavy hitter of Caribbean cruising—energetic, scenic, and frequently crowded. While often marketed solely as a duty-free shopping haven, the island offers substantial rewards for those willing to look past the jewelry stores lining Main Street. The town itself is a steep amphitheater of red-roofed Danish colonial buildings rising from the harbor. For history buffs, a walk up the "99 Steps" (built from ship ballast bricks) or a visit to the red-walled Fort Christian provides context often missed by the average shopper.

Most visitors are here for the water. Magens Bay is the headline attraction—a heart-shaped, protected cove that is visually stunning but often packed with fellow cruisers. For a more relaxed experience with superior snorkeling, we recommend Sapphire Beach or the pristine, protected sands of Smith Bay Park (Lindquist Beach). If your ship docks at Havensight, the Skyride to Paradise Point offers the classic Caribbean vista and a convenient spot for a local Bushwacker cocktail.

Crucial Logistics: St. Thomas has two distinct cruise piers: Havensight (closer to town/Skyride) and Crown Bay (further west). Be sure to check your docking location, as it dictates your walking options. Traffic in Charlotte Amalie can be gridlocked in the afternoons; always pad your return time. Getting around is best done via "safari" taxis—converted open-air trucks. They are efficient and affordable, but confirm the fare before boarding. For those who have visited St. Thomas before, the ferry to neighboring St. John offers a quieter, more natural escape, though it requires careful time management.
